Case Management / Swing Bed

Modoc Medical Centers federally approved Swing Bed Program is a collaborative, interdisciplinary approach to care for patients that are transitioning from a hospital stay to home. After a hospital stay, many patients need additional therapy, support, or medical treatments to heal. The amount of time a patient stays in our swing bed program depends on their individual treatment plan and goals.
 
Skilled care through our Swing Bed Program includes the following services as determined by the individual needs of the patient: 
  • Physical Therapy
  • Wound care
  • IV medications and monitoring 
  • Pain management focused education on the safe use of medications and medical equipment 
  • Case management and discharge planning
  • Dietary services and counseling with a Registered Dietician  
 
Our care team also includes physicians that are available 24/7 to oversee individual care and in-house emergency services, on-site laboratory and imaging services, and Pharmacist consultations.
 
Our goal at Modoc Medical Center is to give our patients the best opportunity to make a successful transition from hospital to home.
